There were failings by Irish Water in the level of customer service it provided to the 7,000 people who were impacted by excess manganese in the Baileborough Public Water Supply.
The incident happened from October to December 2019.
These were the findings of the latest Water Advisory Body report which was published this week.
It stated the excess managanese resulted in water restrictions for those serviced by the plant.
The report also highlighted how Irish Water was subsequently tasked with upgrading the facility.
