A Co Cavan centre for adults with intellectual disabilities was found to be "not compliant" in six areas during an unannounced HIQA inspection in July.
The inspector said the "overall governance and management" of Manderely Lodge "was not effective and there were unsafe and inappropriate practices in the centre".
Manderely Lodge provides 24 hour care and currently accommodates up to five female adults from 18 years upwards, with an intellectual disability.
During the unannounced inspection in July, the centre was deemed to be not compliant regarding staffing, training and staff development, as well as governance and management.
The centre was also not compliant in regards to resident's rights, health care, and medicines and pharmaceutical services.
The inspector reported "significant concerns in the provision of appropriate and safe healthcare and in medicines management".
They said this was "compounded by a lack of oversight by the person in charge and the provider in relation to the healthcare risks which were not being addressed at the time of inspection".
It was also noted that care staff "did not have the necessary skills or knowledge to meet the residents' emerging needs".
As a result, the inspector "was not assured that adequate support was being sought for residents during periods of ill-health".
A not compliant judgment means the provider has not complied with a regulation and considerable action is required.
Where non-compliance poses a significant risk to the safety, health and welfare of residents, it is risk rated "red", and this was the case when regarding Health Care.
Manderely Lodge was requested to become compliant in these six areas by mid-September and a "full medical and healthcare file review" was undertaken in the centre in August.
