A Co Cavan nursing home was found to be "not compliant" in three areas during an unannounced HIQA inspection.
St Joseph's Nursing Home in Lurgan, Glebe, Virginia, was subject to the inspection on the 2nd of September.
St Joseph's Nursing Home provides 24-hour nursing care to up to 52 residents, who require long-term and short-term care.
The inspector found that residents viewed the centre as their home and were content and well looked after.
However, "serious concerns" were raised in relation to what was described as "ongoing governance and management arrangements", as well as a "lack of management oversight".
Although the inspector "identified improvements", it was deemed that there was no "well-defined management structure with clear lines of accountability and responsibility".
The inspector also found that "there was no senior management staff on duty at weekend to provide direct supervision to staff seven days a week".
The centre was also not compliant regarding fire precautions as "three fire doors were not closing" and "emergency evacuation chairs and fire equipment were obstructed by the residents' equipment".
The centre was also deemed not compliant in the area of "Individuals assessment and care plan", as there were "inconsistencies in the risk assessment completed for each resident".
In response, the centre has ensured there is "always a senior nurse on duty at weekends who provides direct supervision to staff and Residents".
New fire doors have also been installed and the evacuation chairs are now in a different location.
The centre says it has also "incorporated a more robust person-centered approach to social activities and social care needs of our residents".
